Visiting a new doctor can be a little scary, but it doesn't have to be. If you prepare ahead of time, you can make sure your first visit is as easy and stress-free as possible. Florida Center for Neurology is pleased to provide various educational materials to help you understand your particular symptoms. You can also fill out patient forms online to ease the new patient process.
Your First Appointment
Please be sure to complete the necessary forms prior to your first appointment, and either submit them here or bring them with you. Here are some important things to bring to your first appointment:
- Completed Forms/New Patient Packet
- Driver’s License or Picture ID
- Insurance Information & Necessary Payment
- If patient is a minor, a parent/guardian with photo ID
- Please read these two documents prior to your first appointment
